I only approach woman at the gym if they have their headphone off. If a girl wants to talk to you in the gym she usually takes them off and works out in your vicinity. If she hits you with the "I have a boyfriend" I always say, "I was asking for your name, not if you had a boyfriend :)" then I end the convo haha

If you're going to the gym regularly, it is likely they are, too. Let the environment do the work for you. Familiarity breeds comfort. A slight nod of acknowledgement and warm smile. The consistency in both your gym visit demonstrate your dedication.
Then, be direct and address the contingency: I think you're nice and would like to know you, and hand her your phone with the dial pad already brought up. Done.
